INSURANCE? If you want to use medical insurance we can possibly get labs covered, with the correct diagnosis codes and assuming your policy covers “out of network” providers on lab orders. Additionally, a superbill is generated automatically through the portal. If you want me to provide the superbill with a diagnosis code for insurance reimbursement than please notify me as the automatically generated superbill will not have this information on it (unless we discuss that you want a superbill). To understand if this applies to your insurance, please contact your insurance company and ask them if they provide reimbursement for ““out of network providers” (they may provide up to 85% of reimbursement for example). Then you would provide them a superbill document attached to your payment receipt, I am able to provide a superbill after the patient requests it from me.

New Patient Visits (75mins) - $280

Includes follow up 15min phone call to discuss response from first visit, as well as to review test results ordered during the first visit. New patients also get discounted labs and supplements, as well as support for questions through the online portal. The free introduction is highly recommended prior to first visit. Returning Patient Visits (25mins) - $120

Includes follow up 15min phone call to discuss response from first visit, as well as to review test results ordered during the visit.

Returning Patient Visits (50mins) - $220

Includes follow up 30min phone call to discuss response from first visit, as well as to review test results ordered during the visit. This longer visit includes prolotherapy/perineural injection treatment, assuming that is decided towards the beginning of the visit (ideally prior to the visit).

Prolotherapy / Perineural Injection Visit and Treatment for 3 or more areas (25mins) - $180

Includes follow up 15min phone call to discuss response from first visit, as well as to review test results ordered during the visit. A longer visit of 50mins covering 4 or more areas is available at $220 (same price as hour visit).

Prolotherapy / Perineural Injection Treatment Only for 1-2 areas (15mins) - $150

Only available for recent prolotherapy visits as a follow up. If you need evaluation by Dr. Vose as well as treatment plan for new condition (not previously treated by Dr. Vose) or would like time to discuss your overall health, then please select the “Injection Visit and Treatment”

IV therapy visit (50mins) - $100 + medication fee

Includes up to 1 hour of time in private IV room with next door bathroom access. Additional time taken may be charged at $50 per hour, after a 15mins grace period.

Lab draw appointment - $35 per 15mins

Includes up to 15mins visit with Dr. Vose answering brief questions, drawing blood, and processing lab specimen for shipment. Please come prepared by drinking enough water and eating a meal prior to most labs (unless specifically a fasting lab draw). Most often fasting labs are drawn at labcorp under insurance for cholesterol and blood sugar evaluation. Fasting is NOT usually required for naturopathic lab evaluation unless diabetes or specified for another condition. Fasting introduces an artificial stressor on the body and changes many labs including TSH under some medical conditions.

Administration hours, per 15mins - $30

Includes paperwork requiring physician authorization such as work release, insurance documentation to substantiate claims, providing detailed collaboration with other providers, providing disability or other evaluations. This must be specifically request and is not available to schedule online.
